Achieve Rehabilitation Services and Supports’ mission is to empower individuals to live the lives they choose to their fullest potential.
Achieve exists to serve individuals who are suffering from severe and/or chronic mental illness and those with intellectual disabilities by helping them to maintain or reintegrate back into meaningful roles in their lives. This goal is accomplished by empowering individuals to realize their potential, putting them in control of their own lives. We do this through skill transference and by helping our clients have the same opportunities as other members of their communities. Achieve emphasizes an individual’s ability to choose how they want to live their life and comes along beside them on their journey.

Our amazing team has over 30 years of experience helping individuals in mental health recovery. Our commitment starts with our staff. We believe that by taking care of our staff they will in turn be able to provide the best care for our clients.

Achieve Rehabilitation Services and Supports has a Continuous Quality Improvement (CQI) plan and is committed to CQI processes. The CQI plan will provide for a bi-annual review of the quality, timeliness, and appropriateness of services including:
Outcomes for Psychiatric Rehabilitation Services
Individual record reviews
Participant satisfaction
Use of exceptions to admission and continued stay requirements.
Evaluation of compliance to the agency service description
Wait times for clients prior to their scheduled appointments.
